Our multidisciplinary team, supported by our administrative team, provides primary health care services for our registered clients. We also provide a wide range of health promotion and disease prevention programs for everyone in our community.
Family physicians practice at the Centre, providing care for our roster clients in a team concept, collaborative approach to primary health care. An appointment is necessary to see a physician. In order to provide continuity of care, you may choose to see the same provider. If that provider is unavailable you may be seen by another member of the health care team. If you have no family physician, and wish to become a client at the Centre, please ask for a roster application form at reception.
A nurse practitioner (NP) is a registered nurse with advanced university education who provides personalized, quality health care. Ontario nurse practitioners provide a full range of health care services to individuals, families and communities. NPs work in partnership with physicians, nurses and other health care professionals to keep people healthy.
The Registered Nurses and Registered Practical Nurses anchors our Clinical Team by supporting both the physicians and nurse practitioners in their practice, as well as coordinating referrals, advocating on behalf of clients, and providing individual client education. Nurses are also involved with many health promotion programs.
You can refer yourself to counselling relating to our dietitian for individual, heart health, weight management or any other nutritional information you may require. Any community member can use this service simply by calling and booking an appointment.
A social worker is available by appointment. You can refer yourself by calling the Centre. The counsellor will return your call to assess need and urgency of your request.
