The following are the priority groups for registering NLCHC primary health care clients:
- Indigenous peoples
- Seniors
- Families with small children
- Farm families
- Living with a disability
- Youth at risk
- People living with moderate mental illness
- Low income individuals/families
Applicants MUST be residents of NLCHC’s catchment area. A map of our catchment area is shown to assist you to determine if you qualify.
Applicants will be prioritized on a waiting list for intake; priority is given to those without primary care. We will be registering priority group applicants without primary care first. Acceptance to our practice (Physician / Nurse Practitioner) will be made on the basis of available openings in the teams’ caseload.
When we select your application, we will send you a letter inviting you to attend a mandatory interview session. At that time, we will explain how the Centre operates and how to access NLCHC’s programs and health care providers. This meeting will help you decide if this is where you would like to come for your health care.
Please be aware that it may take several months for us to consider your application and we would like you to contact us should your health status or personal information change while you are on the waiting list. We appreciate your patience as we go through this process.
