Online Booking

Online Booking for Primary Care is  ONLY available to clients of the CHC. Please select the location you most often see your provider.(Physician/Nurse Practitioner)

Anyone can book an Online 30 minute phone appointment to speak with our Diabetes Education Team by selecting a site and entering all required information to book.
