The Cardiac Rehabilitation Program (CRP) in Sarnia is delivered by the North Lambton Community Health Centre and is part of the regional rehabilitation program based out of St. Joseph’s Health Care London. We are an outpatient program that operates on a referral basis. Our team of health professionals work with clients with known cardiovascular disease to create and follow an individualized exercise rehabilitation program.
Features of the program include:
- Medical evaluation
- Physical activity counseling – clients are offered a 3-month supervised exercise program free of charge
- Support with nutrition and weight management
- Heart health education
- Coordination of patient care
Heart Failure Clinic:
The Heart Failure clinic is an outpatient program that operates on a referral basis. The Nurse Practitioner works with community partners to establish an inter-professional collaborative model of patient care between the Nurse Practitioners, Cardiologists, Internal Medicine Specialists, Primary Care Providers, and existing Allied Health teams to deploy a “hub and spoke” organization of team care. Services are based on individual patient needs, incorporate the latest best practices, and give patients the tools to self-manage their disease.
