WHAT SERVICES DO WE PROVIDE?
Our multidisciplinary team, supported by our administrative team, provides primary health care services for our registered clients. We also provide
a wide range of health promotion and disease prevention programs for everyone in our community based on assessed needs.
Family physicians practice at the Centre, providing care for our roster clients in a team concept, collaborative approach to primary health care. An
appointment is necessary to see a physician. In order to provide continuity of care, you may choose to see the same provider. If that provider is
unavailable you may be seen by another member of the health care team. If you have no family physician, and wish to become a client at the Centre,
please ask for a roster application form at reception.
NURSE PRACTITIONER SERVICES
The Nurse Practitioner (NP) are registered nurses with additional university education to expand their scope of practice - such as prescribing
certain medications. The NPs see clients independently for many health concerns such as baby examinations/immunizations, yearly physical exams, pap
tests and health concerns such as colds, ear infections and bladder infections. The NP consults with the physician in certain health situations.
Health promotion and health education are also key skills that the NPs and physicians bring to client interaction as well as group education
REGISTERED NURSE SERVICES
The Registered Nurse anchors our Clinical Team by supporting both the physician and nurse practitioners in their practice, as well as coordinating
referrals, advocating on behalf of clients, and providing individual client education. Nurses are involved with many health promotion programs
such as prenatal classes.
You can refer yourself to counselling relating to our dietitian for individual, heart health, weight management or any other nutitional information
you may require. Any community member can use this service simply by calling and booking an appointment.
A social worker is available by appointment. You can refer yourself by calling the Centre. The counsellor will return your call to assess need and
urgency of your request. Any community member can use this service.
A registered nurse and dietitian are avaiable to all North Lambton community members to provide a variety of group programs, workshops, and
individual appointments to support those with Diabetes.
